Put the car in third gear and try to take off from a stop. If the engine stalls, the clutch is okay.
If you can release the pedal all the way and the clutch still slips without the car moving, then it either needs to be adjusted or replaced.

Clutch wear.
Directly proportional to the number of times you have to adjust the free play.
Freeplay is the amount of travel of the release fork in the engine compartment, before it contacts the fingers of the pressure plate.
You can test that by moving the release fork with your fingers, bypassing the cable and pedal mechanisms. Freeplay at the pedal should be about 13mm .5 inch minimum. If you have no freeplay the throwout bearing is always in contact with the pressure plate fingers and that could cause premature failure of the bearing.
I haven't noticed any change in 12k miles, seems to be exactly as it was when new, but my clutch is more for gear changes than slipping on hard starts, the benefit of learning initial engagement technique.
