inuvik is right, there haven't been many reported here, but that's just what we hear about. Of all the thousands sold, a small percentage wind up here so who knows the numbers & statistics.
As far as under what operating conditions, you'll have to ask those owners. I don't think they were racing or towing with them, but I could be wrong. Maybe they were always running late to work? I don't know.
I forget - did Neons get Mitsubishi engines?

2017 Mirage - track project and DD
1990 Talon TSi - E3 16G turbo, FIC 980s, 27psi, DSMlink, tons of mods. The heartbreaker.
There are no head gasket issues with these engines.
Each and every case was a case of abuse or high RPM usage. Stock for stock, they are very reliable.
Anyways, Matt I look forward to what you come up with!
Thanks everybody!
Tomorrow we start front sway bar fabrication. The next mod I’ve decided is going to be the manual transmission swap. Stay tuned

Alright everybody I wanted to make a post to update this after having a few days to enjoy my car with the new front sway bar. I cannot even possibly put into words just how profound the difference is with the front sway bar (coupled with the rear of course). It’s unbelievable just how well this chassis responds to well thought out mods. There’s quite literally next to no body roll, and because of the light weight of our cars; this thing rules. Even on the skinny tires it blows my mind how quickly I can get the Mirage down a winding road.
This car is a freaking blast!

Mirage owners look at the world differently than everyone else, but in a better way
We're driving the Beetle of the 21st century, the greatest small car now available!

Wouldn't it be nice to have a get-together to try out some of this stuff. There's more than a few Mirage Forum members in these parts. Even if only a few could make it.
